Pursuant to Article 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (hereinafter “GDPR”), I hereby inform you that:

  2. The Controller has appointed a Data Protection Officer (DPO) – Katarzyna Makarska – who can be contacted in matters related to the protection of your personal data and the exercise of your rights by e-mail at IOD@felgdent.com.

  3. Your personal data will be processed for the purpose of responding to a message sent through the contact form available on the website https://felgdent.com, based on your consent (legal basis: Article 6(1)(a) of the GDPR).

  4. In certain situations, we have the right to transfer your personal data if it is necessary for us to provide our services. We may transfer data to the following categories of recipients: persons authorized by us, our employees and collaborators who must have access to the data in order to perform their duties, processors to whom we delegate certain tasks, and other recipients of data.

  5. We do not transfer your personal data outside the European Economic Area (EEA).

  6. Your contact details will be stored until you withdraw your consent, but not longer than until the end of the calendar year following the year in which you last contacted us.

  7. You have the right to:
    – request access to your personal data and receive a copy thereof;
    – request rectification, erasure, or restriction of processing;
    – object to the processing of your data;
    – exercise the right to data portability.

  8. You have the right to lodge a complaint with the President of the Personal Data Protection Office (UODO) or another supervisory authority if you believe that the processing of your personal data violates the GDPR or the Polish Personal Data Protection Act of 10 May 2018.
